Talent Agreement and Photo/Video Release Form

All work provided by Model Model LLC and it's clients are considered contract. The models are not full or part time employees. This contract can be canceled at any time by either party. David Whitney, and The Midwest Model Agency (Midwest Model LLC) have permission to use any photograph and/or video taken or obtained by David Whitney and Midwest Model or provided by the models and may be used publicly to promote the talent in all states of The United States of America, and international campaigns and products. (It is also understood that no royalty, fee or other compensation shall become payable to me by reason of such use of images and video.) In regards to video and photo web/social buyouts, the rate is also 80/20. It is understood that the images may be used in print publications, online publications, websites, and social media. Some customers will pay for travel time and some do not.
Compensation is 20% for the Agent and 80% for the actor/model per job.
This is an exclusive agreement for 1 year and a 2 year contract that is non-exclusive. (2 years total) David Whitney will act as your agent and represent you to advertisers and producers for modeling and acting and voice over jobs. Any job can be declined. Please arrive on time for jobs. Any work talent gets on their own is subject to no fees or compensation. Any customers you have already are yours not any of our business. After a job is complete, the customer will be invoiced. Payment can take several weeks to arrive. Please alert us to any schedule conflict or emergency. We recommend not doing nude photos or video. We never recommend tattoos on neck or face. We may help with research on modeling inquiries from suspicious parties. For fashion models long colorful fingernails are fun but not helpful for acquiring work. Never stay at a job or situation where you feel you may be in danger of any kind. Always be aware of your surroundings. Always bring a friend or bodyguard with you to jobs if possible.
In regards to the talents own social media, this agency will take no compensation.
Midwest Model provides Head Shots and Photography: Fashion shoots at our studio and other locations at no cost to certain talent. In order to complete a model's portfolio Midwest Model provides if needed (and if the model can travel to the studio.) 1. Head shots, 2. Fashion Shoots, 3, Video Shoots, 4. Audition Shoots, 5. Website web page Profile (Portfolio) 6. Outfits provided during shoots if available. (Printed portfolios are not provided.) Shoots are dependent upon model availability to the studio and schedule.
These shoots are to be used for promotional purposes on websites and the Internet. Websites: midwestmodel.us, tiktok.com, instagram.com, facebook,Rumble and Youtube. Any photos or video supplied by the talent are appreciated. If you post a photo we provided we would appreciate a tag on your social media but it is not required. You agree not to make negative reviews on Social Media concerning Midwest Model Agency or any of the other models or talent for two years. If there is a conflict with other talent or concern please bring it to our attention. All Photos and Videos are co-owned by the signed model and the agency.
Please inform the agency if you become unavailable for work, or you change your appearance. (like a hair cut or color) midwestmodelagency@yahoo.com call or text 515-290-5118. If you use Facebook consider friending David Whitney for the purpose of managing all the talent for events, projects and opportunities. But it is not required. We recommend separate Instagram and Facebook accounts for models for privacy. Models may also use a fake name for privacy. Note: “Promotional name to use”.
